Root canal 5 years ago and now infection

Went to Endodontist last week with pain. Original root canal done in 2017 and never bad a problem since. Endo Diagnosed me with infection and suggested retreatment of MB and Mb2 off #18 with GentleWave. This lesion looks odd, no?

Lesion does look odd. This almost appears like a bone cyst. Did the endodontist take a CBCT scan of the tooth?

Yes she did. She seems to think it is an accessory canal on the Mb root that was missed. I have GentleWave treatment planned for this Wednesday.

Is a cyst a bad thing?

Not necessarily. It just complicates the treatment. I would retreat the tooth, and then follow up to see if healing takes place first

Dumb question - the PDL on the above scan looks “blown out”. Will it go back to normal or is the PDL lost?
