Is the apex of tooth been punctured?

On my last tooth (#2) I had a root canal on December 2023, then again on June 10th. The problem is that every time I tap, it hurts. The primary doctor want to do the permanent filling and crown and I am hesitant because it still hurts. I went to another doctor for a 2nd opinion and was told that looks like the Apex of the tooth have been punctured. Does it look like its been punctured? I have a pressure like feeling constantly as a base which I dont know if its from the crown of the implant or the third one which I was told there is a small gap where food gets inside. But when the pain intensifies, that whole area of the upper teeth hurts, even my face on the side gets a little puffy. Do you think the apex have been punctured? Should I wait to do the permanent filling and crown on my last tooth? Thank you!!

If there are symptoms, I would wait with the crown. There might be small amount of obturation material and/or cement protruding through the apex, but this should not cause such severe pain. Was the treating dentist a specialist endodontist? Perhaps go get an opinion from an endodontist and make sure there is no missed canals or cracks.
